Billing Update: G2211 Now Payable With Home & Residence E/M Visits in CY 2026

CMS is continuing its effort to better pay for the complexity of longitudinal care by expanding use of HCPCS add-on code G2211—a change that will be especially relevant for clinicians providing home-based services. 

Background: What Is G2211? 

Since 2024, CMS has paid G2211 as an add-on code when billed with office and outpatient E/M visits (CPT® 99202–99205 and 99211–99215).
The code is intended to capture visit complexity that is not otherwise reflected in standard E/M valuation, including: 

  • Ongoing management of chronic conditions 
  • Care coordination across settings 
  • The cognitive and relational work involved in longitudinal care 

G2211 is billed in addition to the primary E/M service when the visit reflects ongoing responsibility for the patient’s care. 

What’s New for CY 2026 

Beginning in CY 2026, CMS will allow G2211 to be billed as an add-on code with home or residence E/M visit codes, including: 

  • 99341–99345 (New patient home/residence visits) 
  • 99347–99350 (Established patient home/residence visits) 

This change aligns home-based care with CMS’s broader E/M complexity policy and ensures consistent payment across care settings.
