Denial and claim position
The exact denied line, the stated reason, and whether the route is an appeal or a corrected claim.
Appeal and redetermination review for wound care, debridement, and the documentation a denial puts in issue.
A wound-care denial is usually a record problem before it is a legal theory. Rivet Health Law reviews the denied line against the contemporaneous note, the code that was billed, and the coverage policy that applies to that date of service.

The Challenge
Green means the contemporaneous record appears to meet the requirement, and the appeal should cite the exact location. Yellow means the service may be supportable, but the needed note is missing or unclear. Red means the billed service and the current record should not be defended until the defect is resolved.
The exact denied line, the stated reason, and whether the route is an appeal or a corrected claim.
Type, complicating factors, and a plan of care tied to the wound in the note.
Location, size, depth, and how the wound responded to the last measurement.
Depth billed against the tissue actually removed, and the area that supports the code.
How many excisional debridements are in the record, and whether stalled healing was reassessed.
A separate coverage policy for the product and the date of service. LCD L38902 does not cover this line.

Who We Help
LCD L38902 and Article A58565 are the framework for underlying wound and ulcer care, including assessment, debridement, and utilization. LCD L38902 states that it does not address skin substitutes. A skin substitute or cellular and tissue-based product denial needs the separate coverage policy for that product and date of service.
Our Process
These steps organize the file. They are not a substitute for counsel.
Identify the exact denied line, including the code, units, modifiers, and date of service.
Write down whether the denial is coding, documentation, medical necessity, frequency, or a noncovered service.
Decide whether the route is a redetermination or a corrected claim.
If a skin substitute or CTP is on the claim, pull the policy that governs that product.
Green means the contemporaneous record appears to meet the requirement, and the appeal should cite the exact location. Yellow means the service may be supportable, but the needed note is missing or unclear. Red means the billed service and the current record should not be defended until the defect is resolved.
The color is a triage decision, not a legal conclusion. A red row does not mean the claim is unpayable. It means a corrected claim, a reopening, or another route may fit better than an appeal that defends the claim as billed.
LCD L38902 expressly does not address skin substitutes for wound care. Use it for the underlying wound, the debridement, the documentation, and the coding analysis.
If the denied line is a skin substitute or CTP product or application, identify the separate governing coverage policy for that jurisdiction and date of service before treating L38902 as the answer.
